This is the derailment that occurred in 2021 when someone deliberately
left a vehicle on the track.
I was interested in the report to see what issues they might have had
with the internal emergency door release (IEDR), particularly as regards
the separation of the wiring between the driver's cab and the carriages
while the train was still moving.
However, it turns out that the train had not been retrofitted, even
though the requirement has been marked as "closed subject to
implementation of an approved program or plan."
Quite why a recommendation would be marked closed on such a basis
escapes me. There is a clear risk that a later change of plan would not
be captured, and the ability for the regulator to track the lack of the
IEDR would be lost. I'm not saying this happened here, but the lack of
an IEDR seventeen years after the recommendation is obviously concerning.
